Last year, Patricia Bittiker of West Palm Beach was a down-and-out, self-employed bookkeeper who couldn't afford health insurance. When she felt desperately ill, she would head to a hospital emergency room or join the long lines at the city's 45th Street health department clinic.
Wow, this is a story worth spreading. Good for MDVIP, good for those without insurance.

LEt's rephrase- better service - at a cost. Bravo doctors - you've found a perfect way to blend traditional practice with a added service option. It's great that patients can choose - just like they can in airlines - first class or coach. As long as you get from a to b, the medical care remains equivalent - there are no ethical worries. And unlike MDVIP, you're not using an extortion technique that MDVIP uses - pay up or find another doctor.

Physicians in Retainer (“Concierge”) Practice
This is a 2005 published paper review of the concierge medicine movment published in the Journal of General Internal Medicine. I do disagree in their conclusion. I think they greatly confuse "covered services" versus what is really covered. It makes the assumption that insurnace companies actually pay for the services that we provide. Any patient of a concierge practice will greatly dispute that. Also, the principle of discontinuity- well, it doesn't affect new doctor's brave enough to start off with a retainer practice.
But well worth a read nevertheless.

Physicians who elect to try concierge medicine in an attempt to be free of health plan contracts, should remember that they are not completely free of formal arrangements with payers.
Interesting points when you design your retainer contracts. There are no short cuts. Pay for good legal advice, experienced. Do not just copy someone else's agreement. Having to close your doors is the last you want after going through everything you need to to do practice ideal medicine. See my previous link of experienced attorneys.
